Urine Specimen Collection Procedure

ROUTINE 24 HOUR URINE COLLECTION

The 24 hour urine specimen is to be submitted in a chemically clean, properly labeled urine container. Patients should not be allowed to submit urine specimen in their own “clean” jars. Metro lab adds required preservatives or supplies the proper preservative with the container.

Patient is provided with written instructions for urine collections.

Recommended patient instructions for 24-hour urine collections.

To the patient:  Follow these instructions in collecting your 24 hour urine specimen. You will find it more convenient to void (urinate) into the smaller container provided and transfer the urine in to the larger collection container. Do not add anything but urine to the container and do not pour out any liquid or powder that may already be in the collection container. These substances may cause burn if touched. The collection container should be kept refrigerated through out the collection period. Upon arising in the morning, urinate into the toilet, emptying your bladder completely do not collect the sample. Note the exact time and print it to the container label.

Collect all urine voided subsequently for 24 hours in the container provided. All urine passed during the 24 hour time period (day or night) must be saved. Urine passed during bowel movements must also be collected.

Refrigerate the collected urine between all voiding or keep it in a cool place.

At exactly the same time the following morning void completely again (first time after awakening), and add this sample to the collection container. This completes your24 hour collection

Take the 24 hour specimen to the physician’s clinic /laboratory as soon as possible, maintaining the cool temperature in transit by placing the specimen in a portable cooler or insulated bag.

For the physician/laboratory/ collection centre:

When aliquots are required with preservative, use only the preservative requested. If only a portion of the 24-hour collection is submitted to the lab, be sure to measure the entire 24 hour volume and record the total amount in milliliters (ml) on the test requisition form for laboratory use.

When the lab requires a pooled, well –mixed specimen (collected in more than one container) or a measured aliquot for an individual test, provide the specimen prepared exactly as stated in the specimen requirement submit this to metro lab as soon as possible.

In case if the test request is for two parameters requiring 24 hour urine collection with different preservative requirements then follow the below mentioned procedure:

First 24 hour urine collection (HCl Preservatives )

Urinate in the usual manner on awakening, making sure to empty your bladder completely. Do not save this urine, but you must record the date and time of this first urination. Example: 13/01/2004 7:30 AM

All urine passed during the remaining 24 hour period must be collected in this first container, labeled “HCL Preservative”. Urine passed during bowel movements must also be collected.

Urine may be collected in another clean container and then carefully poured into the first 24 hour collection container. 

Second 24- Hour Urine Collections (No Preservative)

After the first 24 hours collection procedure is completed.

For second collection start from step 1 to step 4 as given in first 24 hours collection accept the urine must be collected in a container without preservative. Labeled “No Preservative”

Blood Specimen Collection Procedures
Coagulation assays (For PT, APTT and Fibrinogen)
  • Confirm if patient is taking any medicines/drugs. If yes then note down the names and doses.
  • Confirm if patient is suffering from any other disorder like Thrombosis etc.
  • One pair of sodium citrate (3.2%) should be mixed with 9 parts of whole blood.
  • For sodium citrate tube of 2ml 1.8ml of blood(Up to the mark given on tube)
  • For sodium citrate tube of 3ml2.7ml of blood(Up to the mark given on tube)
  • After collecting the venous blood in the sodium citrate tube, mix it by gently inverting the tube 3 to 4 times
  • Spin the sample at 3500 rpm for 20 minutes to separate the clear plasma.
  • Separate the clear plasma in a plane plastic vial and again centrifuge for 15 minutes.
  • Immediately pack the separated plasma in the ice pack and dispatch.
  • If to be processed after 6 to 8 hours then store in the freezer at -20C.

If sample is from outstation then frozen plasma- containing vial must be packed in the sample Styrofoam box in dry ice.

Collection time should be mentioned on the respective vial.

Coagulation assays (For factor assays, Activity assays and Lupus anticoagulant.

Platelet poor plasma (PPP) is must for assays

The obtained plasma (by following the first five steps mentioned above) must be centrifuged again at 3500rpm for 15 minutes. The supernatant (PPP) should be transferred to properly labeled plastic vial.

For factor assays like Factor V, Factor VIII, Protein C and S activity, citrated plasma should reach at metro lab in FROZEN state (refer the step number 8 and 9 mentioned above)

Note: - If sample quality is unsatisfactory then repeat sample can be asked.
